Introduction: Due to the increasing number of aging population in developing countries such as Iran and the role of social capital and successful aging in the aging population, this study aimed to investigate the relationship between social capital and successful aging in the aging population, Iran.
Materials and Methods: This cross-sectional descriptive study was performed on 402 aging people randomly selected from 6 health centers in the city, 2 aging homes and 3 open centers (medical sciences, education, social security) in 2019-2020 as a research population by stratified sampling. To collect information from the aging demographic information questionnaires, Onyx and Bowen Social Capital Questionnaire, Successful Aging Questionnaire, and aging Cognitive Status Questionnaire (AMT) were used for data collection. Data were analyzed using SPSS-22 software and Pearson correlation, focal tests, and multivariate regression.
Results: The mean of age of the subjects were 68.8 years. The social capital score was correlated with the successful aging, which was positively and moderately correlated (r = 0.662). Nearly 30% of the changes in the scores of the dependent variable of successful aging were influenced by the variables of social capital and marital status, living with relatives, employment, and educational level (r2 = 0.29).
Conclusion: The results of this study confirming the predictive power and impact of social capital on the index of successful elderly in the community are puzzling and relationship was found to be more significant. Social capital as an independent variable significantly affects successful aging as a dependent variable.in other words, social capital has the ability to predict successful aging.in some aspects including physical health, mental health, physical function and cognitive status. This findings could be a good guide for further research focus and for the authorities and practitioners, to achieve successful aging goals.

Background & Objectives: By knowing of minimal knowledge in the diabetics and by attending to the high prevalent of effects and low quality level of life in the diabetic patient, discovering an appropriate educational method is important. The purpose of this study is comparative assessment of effect of education in the OREM`s self-care model way with current method on the quality of life in diabetic type 2 patients.
Materials & Methods: This study was done on the base of randomized clinical trial method on diabetic type 2 referrals to diabetic clinics of Sabzevar city at the year of 1392. Control group were under the current education of diabet center and the case group was trained by lectures, question and response, group discussion, films and educational notes. One month after ending education, all of research units were assessed using special measure of quality of life in diabetics (ADD QOL 19). Data were analyzed by SPSS software and kai - square, mann-whitney, co-variances and logistics regression exams.
Results: In this study 50.7% (38 person) were women and 49.3 (37 person) were men. After the educational implementation, mean and SD score of QOL in the control and case groups respectively were -1.48 ± 0.719 and 1.815 ± 0.931 in the physical dimension; -1.9 ± 0.643 and 2.22 ± 0.778 in the psycho spiritual dimension; 1.48± 0.719 and 1.85 ±0/931 in social dimension. After the educational implementation, in the co-variances exam meaningful differences were seen between mean scores of QOL of two groups.
Conclusion: Unlike the current method, education plan of OREM`s model extolled the QOL of diabetics. Therefore, this method is recommended for use.

Background and Purpose: Most corticosteroids are dose- and duration-dependent. Dexamethasoe has been the highly used drug in the country in the last years, and can produce a pseudo-kushing syndrome if use in the long run. In the present paper, we will report a case of dexamethasone use for twenty years. The Patient: The patient is 52-year old man, married but has no child; he has started injecting dexamethasone 30 years ago; he keeps cattle and has a farm too. The patient was hospitalized first with a diagnosis of drug-resisitant brucellosis in Jovein, Iran. Laboratory tests indiated anemia, leukocytosis, hypocalemia, hypocalcemia and hyponathremia; he was discharged from the hospital four days later. Two days later, he was referred to neurocerebral surgeon due to backache, lower and upper limbs numbness, and walking disorder with a diagnosis of spastic quadriparesia and babinsky. Sixteen days after that, he underwent a total laminectomy surgery of C3-C6 with the diagnosis of cervical myelopathy. He had already been hospitalized in Mashad, Iran for a fever of unknown causes two years before that, and cataract surgery of both eyes. Conclusion: Long-term use of dexamthasone has caused six hospitalizations, diagnostic measures, medical interventions and surgical procedures for the patient reported here.

Background and Purpose: Situs inversus or situs transversus is a rare congenital condition where abdominal organs are positioned in a transverse manner, as in front of a mirror. Its prevalence is less than one in 10/000. This paper is intended to report one such case.
Patient's history: A fourteen-year-old boy referred to the clinic with complaints of leftward abdominal pain beneath the navel, nausea and vomiting. Early diagnosis of acute appendicitis was made after clinical examinations, laboratory tests and sonography. Radiography reports revealed the shift of heart to the right of chest, confirming the diagnosis of Dextrocardia.
Gastric bubble and spleen were observed on the right and liver and cholecyst on the left with sonography. Pancreas was also positioned on the opposite side. LLQ results indicated acute appendicitis on the left. Clinical findings and paraclinic measures rendered the certain diagnosis of situs inversus totalis. The patient finally underwent left appendectomy.
Conclusion: Normally, situs inversus is accompanied with dextrocardia and major vessels dislocation and its prognosis is good. This condition with levocardia is less common and amounts to one in 22/000, with a likelihood of increasing congenital cardiac diseases. In the reported case, no particular problem came up after surgery. Inverted anatomical condition and the atypical history of situs inversus make the early diagnosis difficult in these patients. Accurate history and clinical examinations and reliance on imaging techniques in traumatic cases can reduce and minimize medical errors in treating such rare cases.
